They're not judging how good the rappers are, but how popular they are.

They''re not judging how popular they are, otherwise it'd be Em, Wayne, Jay, etc. at the top.

They're judging the how popular / hot they've been over the past year. Which rappers are the current "cool thing" to listen to.

Surprised Macklemore didn't make the list at all.

Pretty sure Macklemore didn't make the list because he didn't do a lot of things in 2012, his single became a hit last month. If anything, he's a potential contestant for next year.
