We know Eminem stays away from computer because of porn. But, now he clearly admits that he fell victim to it. He's on his last breath because he's jacking off extremely "HARD" and when you do that you're bound to lose your breath. But, that can also kill you. Also, notice how Em keeps referencing males (Bruce Willis, David Carradine, God and Jack[ing]). Possible hint that the internet porn Em was watching was Gay porn.

We already know Em is a masochist (thread about it coming soon) so this was expected. Em tends to express the things that he wants to do to himself by rapping about doing it to someone else or by rapping from another perspective seeing the violent acts happening to someone else. Examples: the song "Kim", "jumped up, choked a giraffe, snapped its neck and broke it in half" AND "I put both hands on your throat, I sit on top of you squeezing til I snap your neck like a popsicle stick". So, this is just another occasion. "What part you don't get" is a little joke at the end of this huge and highly revealing and puzzling metaphor which just asks the listener what part of it all they didn't quite comprehend. LOL. Silly Marshall. So, in the end Eminem dies hard from all this. But, why is the dying hard when Bruce Willis and David Carradine doing these acts? Because Eminem revealed that it was him but it seems that no one caught that one. In the end, this metaphor depicts Eminem as a perverted, questionably homosexual man who suffers from depression and resorts to internet porn to solve his problems but THAT's what ruins his life and what will continue to ruin his life.
There's probably more metaphors with hidden messages that let us into Marshall's personal life even more. But, this one had to be said.
