Actually I just listened to the song again on my headphones and almost broke my eardrums, but I think Em is also screaming in the background of about half of the song - from 'chloroform rag' to just before the police part. It's really quiet and muffled, like Em is gagged.

I think the whole thing is a dream/nightmare though, until the fourth verse.

this is what i had assumed as well. since it's the only way the whole story/concept could be so vivid to him through the track(s), and then the transition into the end is his subconscious entering his conscious/aware mind, stripping back the whole stan/matthew facade to reveal what it really was after his trip/dreamstate.

edit: and actually this theory fits right in with Uno's new find here, like his subconscious knew it was about to transfer this into what it means in reality.
