If there will ever be a nuclear war, it will throw us back into the middle ages indeed. But South Korea has ended their research for nuclear weapons since the 70's. You forget about Iran though, there are strong suspicions they are actually setting up a big programme involving nuclear research at the moment.
If one initiates a war with nuclear weapons this means death and terror for a major part of world population and we all know that. I doubt a country with those kinds of weapons will initiate a war with those against another country who has them too. That is basically the same as sucide for them. In a situation as the world is at now no country will be using those, something really disturbing in the balance of power needs to happen to make this possible. One example can be when we run out of oil. In that case the U.S., Western Europe, China, Japan and Russia will lose power if investments in independancy of oil stay this low and economic balance will shift to africa and parts of asia and south america where they arent as dependant on fossil recources. In that case I think strange things will happen with a (nuclear)WW3 as a result...
